In this post, I explain the differences between local and general anesthesia, how each type works, which procedures typically use each option, and who may need to avoid general anesthesia for safety reasons.

What Is Anesthesia? Understanding the Basics

Anesthesia refers to the medications used to block pain during surgery or medical procedures. When discussing the details about anesthesia, it is important to know that anesthesia does more than eliminate pain. Depending on the type used, it can control anxiety, memory, consciousness, and movement.

Modern anesthesia is extremely safe. At First Coast Plastic Surgery, I work very closely with anesthesiologists to tailor the anesthesia to each patient’s needs and medical history.

Local Anesthesia

Local anesthesia numbs only the specific part of the body being treated. When discussing the details about anesthesia, I explain that local anesthesia is one of the simplest options available. I commonly use local anesthesia for small or minimally invasive procedures because it only blocks nerve signals in a targeted area.

Local anesthesia for a plastic surgery procedure is usually delivered through an injection. You remain fully awake. With some patients, I also offer oral sedation to help reduce anxiety.

How Local Anesthesia Works

Understanding the details about anesthesia includes knowing how local anesthesia numbs an area. Local anesthetic medications block sodium channels in nerve cells. This prevents the nerves from sending pain signals to the brain.

Because the medication works only on a small, localized group of nerves, the rest of your body, including your breathing, heart rate, and cognitive function remain unaffected. This is why local anesthesia is considered very safe and offers a fast recovery with little if any side effects.

Local anesthesia offers many advantages:

Plastic Surgery Procedures That Can Be Performed with Local Anesthesia

Understanding the details about anesthesia helps patients know which procedures can be paired with local numbing only. Many small or surface-level cosmetic procedures do not require deep sedation or general anesthesia. Common examples include:

1. Minor Eyelid Surgery, (Upper Blepharoplasty)

Upper eyelid lifts can often be done using only local anesthesia because the tissue is thin and the procedure is brief.

2. Liposuction of Small Areas

Very small targeted areas, such as under the arms, or flanks, may be performed with local anesthesia.

3. Skin Lesion Removal and Scar Revision

These superficial procedures rarely require anything more than local numbing.

4. Ear Pinning, (Minor Otoplasty Adjustments)

Some minor revisions can be safely completed using local anesthesia.

5. Small Excisional Procedures

Removal of cysts, benign growths, or small lipomas can be done safely with local anesthesia.

Not all patients are ideal candidates for local anesthesia, especially those who experience high anxiety, have complex medical conditions, or are undergoing a larger or more invasive procedure. This is why I discuss anesthesia in detail with new patients.

General Anesthesia

General anesthesia is the option most people think of when they imagine surgery. With general anesthesia, you are completely unconscious, unable to feel pain, and unaware of your surroundings.

Discussing the details about anesthesia must include understanding that general anesthesia also involves controlling your breathing through a breathing tube or laryngeal mask airway. A licensed anesthesiologist will be present during procedures where general anesthesia is used to monitor and adjusts your vital signs continuously.

How General Anesthesia Works

General anesthesia uses a combination of intravenous medications and inhaled gases. These agents work throughout the brain and central nervous system, causing a controlled, reversible unconscious state.

When learning the details about anesthesia, it’s helpful to know that general anesthesia affects memory formation, pain perception, muscle movement, and awareness. It ensures that you remain completely still and comfortable during your surgical procedure.

General anesthesia offers several advantages, especially for longer or more invasive procedures:

  • Ability to perform extensive surgical work

Plastic Surgery Procedures That Typically Require General Anesthesia

When reviewing the details about anesthesia, it becomes clear why many plastic surgery procedures require complete unconsciousness. These procedures involve deep tissues, muscles and significant surgical work, making general anesthesia the safest choice. Examples include:

1. Tummy Tuck, (Abdominoplasty)

This procedure involves tightening abdominal muscles and removing excess skin, which requires deep anesthesia and complete stillness.

2. Most Breast Procedures

Breast augmentation, breast lifts, and breast reductions almost always require general anesthesia due to the depth of tissue involved.

3. Facelift and Neck Lift

Although mini-facelifts can sometimes be done with local anesthesia and sedation, most facelift and neck lift procedures require general anesthesia.

4. Body Liposuction

Large-area liposuction or multiple treatment areas are safer and more comfortable under general anesthesia.

5. Brazilian Butt Lift (BBL)

General anesthesia is necessary because this procedure involves extensive liposuction and fat transfer.

6. Rhinoplasty, (Nose Surgery)

Although some minor nasal procedures can be performed with local anesthesia, most rhinoplasty surgeries require general anesthesia.

7. Combination Procedures

Combined procedures such as mommy makeovers that are often comprised of breast surgery, abdominal surgery and liposuction always require general anesthesia for patient safety and comfort.

Patients Who Should Avoid General Anesthesia

Even though general anesthesia is extremely safe, some patients may need to avoid it or undergo additional precautions.

I usually do not approve patients for procedures that require general anesthesia if they have the following conditions:

Patients with Severe Heart Disease

General anesthesia places additional demands on the cardiovascular system, which can be risky for those with severe heart conditions.

Patients with Significant Lung Disease

COPD, severe asthma, or other respiratory disorders may increase complications related to breathing under general anesthesia.

Patients with Uncontrolled Sleep Apnea

Sleep apnea can complicate airway management while unconscious.

Patients Who Are Extremely Obese

Obesity can make airway management more difficult and may increase the risk of anesthesia complications.

Patients with Severe Allergies to Anesthesia Medications

True allergies to anesthesia medications are rare, but they do occur.

Patients with Previous Serious Anesthesia Reactions

General anesthesia is not safe for patients with a history of malignant hyperthermia or severe anesthesia complications.

Patients with Certain Neuromuscular Disorders

Some conditions increase sensitivity to anesthesia medications or muscle relaxants.

It is very important for patients to let me know of all medications and supplements that they are taking and any medical conditions they have during their initial consultation. During your consultation, I will review any medications or supplements that you are taking, your medical history, allergies, any previous anesthesia experiences, and any chronic conditions that you may have.

In some cases, I may request lab work, heart tests, a sleep apnea screening, or additional physician clearance depending on your health. This evaluation ensures that your anesthesia plan is personalized and safe for you specifically.

Recovery After Local vs. General Anesthesia

Part of knowing the details about anesthesia is understanding what happens after surgery. Recovery after local anesthesia is typically quick. Most patients can drive themselves home if no sedative medication was used.

Recovery after general anesthesia takes longer. You may experience grogginess, nausea, sore throat, chills, or dizziness for several hours. A responsible adult must drive you home, and you should avoid driving and major decisions for 24 hours.
